Course Overview
This 1-2 Day course is designed to develop the competencies required to conduct effective incident investigations and determine root causes for occupational health & safety incidents in alignment with ISO 45001:2018 or any occupational health & safety management system. The focus shifts from just identifying the symptoms to diagnosing the root systemic causes and implementing preventive strategies to avoid reoccurrence.
Through real-life case studies, class active engagement, 2-way interactions, practical examples, and proven tools, participants will learn how to build a culture of learning and accountability, kickstart prevention-based OHS management leading to continual improvement, OHS performance improvement, OHS due-diligence, and ultimately sustainable OHS.
Learning Objectives
- Explain the role of incident investigation within ISO 45001:2018 and occupational health & safety (OHS) compliance.
- Conduct systematic evidence collection and fact-finding during incident investigations.
- Distinguish between immediate, contributing, and root causes of incidents.
- Apply various root cause analysis (RCA) techniques (e.g., 5 Whys, Fishbone, Fault Tree Analysis).
- Develop effective and sustainable corrective and preventive actions (CAPAs).
- Integrate investigation outcomes into the organization’s risk management and continual improvement processes.
- Prepare clear, objective investigation reports.

Certification
Participants who successfully demonstrate competence during this course will receive a Certificate of Attainment covering the following competency units:
- JSA Inc – II-RCA: Incident Investigation – Root Cause Analysis (OHS)
Participants not deemed competent are issued Certificates of Attendance and given the opportunity to retake the mandatory competencies previously missed.

Participants receive a package that consists of II-RCA (OHS) student manual and other value-added reference materials.
