Incident Investigation and Root Cause Analysis for Quality Non-Conformances

Proudly Canadian. Globally Aligned.

Overview

This 1-2 Day course is designed for quality professionals, engineers, and managers seeking to develop expertise in incident investigation and root cause analysis (RCA) within a quality management framework. Participants will learn systematic methods to identify, analyze, and eliminate root causes of quality issues, defects, and process failures. Prioritizing proven methodologies, the course combines theoretical concepts with real-life case studies, interactive exercises, and tools like 5-Why, Fishbone Diagram, Pareto Analysis, and Fault Tree Analysis. The core objective is to address and improve the processes that initially cause the event, promoting a preventive rather than reactive approach. Ultimately, the corrective/preventive actions lead to continual improvement and sustainable development.

Learning Objectives

  • Understand the importance of incident investigation and root cause analysis in quality management.
  • Differentiate between symptoms, causes, and root causes of quality issues.
  • Apply various RCA tools such as 5-Why, Fishbone Diagram, Pareto Analysis, and Fault Tree Analysis effectively.
  • Conduct thorough investigations of quality incidents and defects.
  • Develop and implement corrective and preventive actions based on root cause findings.
  • Analyze the influence of human factors, process factors, system factors, and defect types in root cause analysis.
